COVID Vaccination Mandate Policy - EO 283 on Paycom

All employees should have received an email from Paycom to advise you to log into your Paycom Account to view a new “Checklist” that has been assigned to you. This “Checklist” is labeled “COVID Vaccine Mandate – EO 283” and includes:

• A copy of the COVID Vaccination Mandate Policy – Executive Order 283 (which is the same information that Melonie Garron sent out via email on Saturday, January 22, 2022) ; and
• Acknowledgment of Receipt of the COVID Vaccination Mandate Policy – Executive Order 283.

VERY IMPORTANT – IN ORDER TO COMPLETE YOUR CHECKLIST YOU MUST PROVIDE AN ELECTRONIC SIGNATURE ON THE ACKNOWLEDGMENT FORM. THE CHECKLIST HAS TWO ITEMS TO COMPLETE.

Please log in to your Paycom Account and access the Checklist in the Paycom Checklist Link – See below for a screenshot.

Please review the new Policy and sign the Acknowledgement Form by no later than Tuesday, February 1, 2022.

All Employees are required to complete this Checklist on Paycom.
