


COVID Booster Documentation Due May 6, 2022

As per the New Jersey Department of Health, AMI is required to ensure staff provides proof that they are up to date with their COVID-19 vaccines — meaning they are fully vaccinated and have received a COVID-19 booster shot, when eligible.
- Documentation of COVID vaccination to include booster dose for all staff eligible to receive must be submitted to AMI by May 6, 2022.
- Updated cards should be sent immediately following receipt to: covidvacc@aminj.com
- Medical or religious exemption requests for the booster vaccine should have been previously submitted to AMI by March 11, 2022.
- Requests for medical or religious exemptions should be submitted to - exemption@aminj.com
- Weekly testing will begin the workweek of May 15th and results this week will be due no later than May 19th.
- Weekly testing is required for all onsite staff who have submitted and received approval of a medical or religious exemption
In accordance with Executive Orders- 252, 283 and 290, notices of noncompliance will be issued to booster-eligible employees who have not yet verified their receipt of a booster by May 11, 2022 deadline. For those who do not meet this deadline, further action, including discipline, will be taken. Please review the updated disciplinary policy sent previously by Melonie.
